This website is operated by City Prime Care to share information about our clinics, services, doctors and appointment options. You may use it for personal, non-commercial purposes, such as learning about our practice, requesting an appointment or contacting a clinic.

When you use the site, you agree not to misuse it. That means you will not try to gain unauthorized access to our systems, interfere with how the site works, upload harmful code, copy content in bulk with automated tools, or submit false or misleading information through our forms.

We may change, suspend or remove any part of the website at any time. We may also update these Terms of Use; the last updated date on this page shows the current version, and continued use of the site after a change means you accept the revised terms. The content on this website, including service descriptions, blog articles and answers to frequently asked questions, is provided for general information only. It is not medical advice and is not a substitute for an evaluation by a qualified clinician who knows your history.

Reading the site, sending a message or submitting an appointment request does not create a doctor–patient relationship. That relationship begins when you are seen by one of our clinicians. Do not delay seeking care, or disregard advice from your own doctor, because of something you read here.

If you think you may be having a medical emergency, call 911 or go to the nearest emergency room right away. Examples include chest pain, difficulty breathing, signs of a stroke, severe bleeding or thoughts of harming yourself. Do not use website forms or email for urgent needs, because messages are not monitored around the clock. For a mental health crisis, you can also call or text 988.

You can request a visit through our online appointment page or by calling either clinic. A request submitted online is not confirmed until our front desk contacts you with a date and time. Please bring a photo ID, your insurance card and a list of your current medications to each visit.

If you cannot keep an appointment, we ask that you give at least 24 hours' notice when possible by calling the clinic where you are booked. Early notice lets us offer that time to another patient who needs to be seen, and it lets us find you a new slot sooner.

If you are running late, please call ahead. Depending on the schedule, your visit may need to be shortened or moved so that other patients are seen on time. Insurance coverage and copays depend on your health plan, so we recommend calling to confirm your plan before your first visit.

The text, graphics, logos, photographs, page designs and other content on this website are owned by City Prime Care or used with permission, and are protected by United States copyright and trademark laws. The City Prime Care name and logo are our trademarks.

You may view, download or print pages for your personal, non-commercial use, such as keeping a copy of preparation instructions for a visit. You may not copy, modify, republish, sell or distribute our content, or use our name or logo, without our prior written permission.
