Our Non-Discrimination Policy
City Prime Care complies with Section 1557 of the Affordable Care Act and other applicable federal civil rights laws. We do not discriminate against, exclude or treat anyone differently because of race, color, national origin, age, disability or sex. This includes discrimination based on pregnancy or related conditions, sexual orientation, gender identity and sex stereotypes.
We also follow New York State and New York City human rights laws, which protect additional characteristics, including creed, marital status and citizenship status.
This policy applies to every part of our practice, including scheduling, check-in, clinical care, billing and communication through our website. Everyone who works at City Prime Care, from front desk staff to clinicians, is expected to treat patients, caregivers and visitors with equal respect. We serve patients from many backgrounds at both clinics, and fair treatment is a basic part of good care.
Language Assistance Services
If English is not your preferred language, City Prime Care provides language assistance services free of charge, including qualified interpreters and translated written information. Our Elmhurst care team speaks English and Spanish, and our Chinatown care team speaks English, Chinese and Spanish. For other languages, we arrange a qualified interpreter by phone or video. You do not need to bring a family member to interpret, and we will not rely on a child to interpret except in an emergency.

Auxiliary Aids & Services
City Prime Care provides free auxiliary aids and services to people with disabilities so they can communicate with us effectively.
These include:
- Qualified sign language interpreters, arranged in person or by video
- Written information in other formats, such as large print or accessible electronic documents
- Calls through relay services, including 711 Relay, for people who are deaf, hard of hearing or speech-disabled
- Extra time and help completing forms
Please tell the front desk when you book which aid or service you need, so it is ready for your visit. We also make reasonable changes to our policies and procedures when needed to give people with disabilities equal access to care. There is no charge for any of these services. How to File a Grievance
If you believe City Prime Care has failed to provide these services or has discriminated against you in another way on the basis of race, color, national origin, age, disability or sex, you can file a grievance with us. Contact our Civil Rights Coordinator through the front desk at either clinic: Elmhurst at (718) 555-0175 or Chinatown at (212) 555-0180. You can file a grievance in person, by phone, in writing or through our contact page, and staff can help you file it. We will review it promptly and respond, and we will not retaliate against anyone who files a grievance.
