Finding doctors that accept Medicaid in NYC should not take a dozen phone calls. City Prime Care accepts New York State Medicaid, including fee-for-service Medicaid, along with the Medicaid managed care plans listed below, at both our Elmhurst, Queens and Chinatown, Manhattan clinics. That makes us a practical choice if you need a Medicaid doctor in NYC for checkups, sick visits or chronic condition follow-ups.
We also see patients with Medicare. As a Medicare doctor in NYC, our team provides the Annual Wellness Visit, preventive screenings that Medicare covers and ongoing care for conditions such as diabetes and high blood pressure. If you have both Medicare and Medicaid, bring both cards so our front desk can bill each one correctly. If you are enrolled in a Medicare Advantage plan, call to confirm that your specific plan includes our clinic before you book.

Medicaid Managed Care Plans
Most New Yorkers with Medicaid are enrolled in a managed care plan rather than fee-for-service Medicaid. Your plan's name appears on your member ID card, and many cards also list an assigned primary care provider. City Prime Care accepts the three Medicaid managed care plans below. If your card names a different primary care provider, you can usually ask your plan to change the assignment to a City Prime Care doctor, and our front desk can walk you through that call. Whichever plan you have, call to confirm your specific plan before your first visit, because coverage can differ between plan types.
Fidelis Care
Fidelis Care members can use City Prime Care for routine primary care, including annual physicals, sick visits and follow-up care for ongoing conditions. When you come in, bring your Fidelis Care member ID card, a photo ID and a list of your current medications. If the primary care provider on your card is not one of our doctors, our staff can help you update it. Call the clinic to confirm your specific plan before scheduling.
Healthfirst
We accept Healthfirst Medicaid managed care at our Elmhurst and Chinatown clinics. Like many insurers, Healthfirst offers more than one type of plan, so the exact plan name printed on your card matters. Read it to our front desk when you call, and we will check whether we can see you under that plan. At your visit, bring your Healthfirst ID card, a photo ID and, if you recently switched plans, the newest card you received.
MetroPlus Health
MetroPlus Health members are welcome at both City Prime Care locations. Before your appointment, look at your member card to confirm the plan type and the primary care provider on file, then call us so we can verify your eligibility for the visit date. Medicaid eligibility is renewed periodically, so mention any recent renewal notice when you call. Bring your card, a photo ID and your medication list to the visit.
Commercial Insurance Plans
Many working New Yorkers get coverage through an employer, a union or the NY State of Health marketplace. City Prime Care accepts a range of commercial plans at both clinics, so the same practice can handle your physicals, sick visits and primary care services whether you live in Queens or Lower Manhattan. Commercial insurers often sell several versions of a plan, such as HMO, PPO and EPO products, and copays and deductibles vary between them. Your card and your plan's member portal are the best sources for those details, and our front desk can confirm whether we are in network for your specific plan.

Aetna
If you have Aetna coverage, bring your Aetna member ID card and a photo ID to your first visit. Look on the card for your plan type and your copay for a primary care visit, and check whether your plan asks you to choose a primary care provider. Ask Aetna how your plan covers an annual physical. Call us to confirm your specific plan before you book.
Cigna
Cigna members can book primary care at City Prime Care in Elmhurst or Chinatown. Before your appointment, sign in to your Cigna account or call the number on your card to review your deductible and primary care copay, since those amounts depend on the plan your employer chose. Bring your card and a photo ID, and tell us about any secondary insurance. Please call to confirm your specific plan first.
UnitedHealthcare
We accept UnitedHealthcare at both clinics. Plan designs vary widely, and some require you to pick a primary care provider or get a referral before seeing a specialist. When you call, give us the member ID and group number from your card so we can check your benefits ahead of time. On the day of your visit, bring the card and a photo ID. Call to confirm your specific plan before scheduling.
Plans by Location
Every insurance plan on this page is accepted at both of our clinics. Whether you visit our Elmhurst, Queens clinic near the 74 St–Broadway station or our Chinatown, Manhattan clinic on Catherine Street, the same Medicaid, Medicare and commercial plans apply, and you can move between locations without changing your coverage. Networks can change during the year, though, so call the clinic you plan to visit to confirm your specific plan: (718) 555-0175 for Elmhurst or (212) 555-0180 for Chinatown. Our staff can also tell you which location has the soonest opening and help you book a visit there.
Uninsured / Self-Pay
Not having insurance should not keep you from seeing a doctor. City Prime Care sees uninsured patients on a self-pay basis at both clinics. What you pay depends on the type of visit and any tests involved, so ask our front desk for current self-pay rates when you call, before your appointment is booked.
If you might qualify for coverage, it is worth checking. NY State of Health, the official state marketplace, is where eligible New Yorkers can apply for Medicaid, the Essential Plan, Child Health Plus and private plans. Eligibility is based largely on income and household size, and free enrollment help is available in many languages. Once you are enrolled in a plan we accept, you can keep seeing the same doctor here without starting over.
How to Verify Your Coverage
A short check before your visit can prevent a surprise bill. Whether you have Medicaid, Medicare or a commercial plan, here is the simplest way to confirm coverage.
- Find your member ID card and note the plan name, member ID and group number, if there is one.
- Call the clinic you plan to visit and read those details to our staff, who will check your eligibility for the visit date.
- Call the number on the back of your card and ask your plan about your copay, deductible and any referral rules.
- Bring your card, a photo ID, your medication list and any secondary insurance card to your appointment.
