Referral Services in Queens & Manhattan, NY

City Prime Care offers Referral Services at our Elmhurst, Queens and Chinatown, Manhattan clinics. When you need a specialist referral in NYC, your primary care provider decides whether a specialist is the right next step, sends the paperwork and records, and stays involved after the visit. It helps anyone whose health question goes beyond primary care, and it keeps all of your care connected in one place instead of scattered across offices.

How Specialist Referrals Work

A specialist referral in NYC can feel like a maze of phone calls, forms and waiting rooms. Our job is to shorten that path. A doctor referral to specialist care starts with a real conversation in our office, not a slip of paper handed across the desk. Your provider reviews your symptoms, history and any test results, then explains why a specialist makes sense and what that visit should answer.

From there, our front-desk and referral staff handle the logistics: completing the referral, attaching the right notes, and checking what your insurance plan requires. You leave knowing who you are being sent to, why, and what happens next. The three steps below show how the process usually unfolds at both clinics.

Step 1 – Visit Your Primary Care Doctor

Every referral begins with a visit at our Elmhurst primary care clinic or our Chinatown office. Your provider takes a history, examines you and may order bloodwork or other tests first, because many questions can be answered without a specialist at all. If a specialist is needed, arriving with recent results often makes that first specialty appointment more useful. Bring a list of your medications, any past reports and your insurance card so nothing slows the next step down.

Step 2 – We Send the Referral

Once you and your provider agree on a plan, our team prepares the referral. That includes a clear reason for the visit, relevant visit notes, medication lists and test results, shared securely with the specialist office. If your plan needs a formal referral number or prior approval, we submit that request as well. You receive the specialist's contact details, and our staff can help you find an appointment time that works around your schedule.

Step 3 – Follow-Up Together

A referral is not the end of our involvement. After you see the specialist, we ask for their consultation notes and review them with you at a follow-up visit or by phone. Together we sort out new prescriptions, check for drug interactions and decide who manages which part of your care. If the recommendations are unclear or you have questions, your primary care provider is the person to call first.

Specialists We Refer To

Primary care covers a wide range of problems, but some conditions need a focused expert. We refer patients to independent specialists and outside facilities based on medical need, your insurance network and, when possible, location and language. We do not own these practices, and the final choice of specialist is always yours.

Common referrals from our clinics include:

  • Cardiology for chest pain, abnormal heart rhythms or hard-to-control blood pressure
  • Gastroenterology for ongoing digestive symptoms and screening colonoscopy
  • Dermatology for skin changes, persistent rashes or suspicious moles
  • Orthopedics for joint, bone and back problems that do not improve
  • Endocrinology for complex diabetes or thyroid conditions
  • Imaging centers for X-rays, ultrasound, MRI, CT and mammograms, which are done off-site

Other specialties, such as neurology, urology or eye care, are arranged as needed. If you already have a specialist you trust, tell us, and we will send the referral there whenever your plan allows it. Some referrals follow results from in-office diagnostic testing, which helps the specialist start with a clearer picture.

Insurance Referrals & Prior Authorization

Many health plans, especially HMO and some Medicaid managed care plans, require an insurance referral from primary care before they will cover a specialist visit. Others, such as many PPO plans, let you book a specialist directly. Rules vary by plan and can change each year, so we check your coverage before sending anything.

Some tests and procedures, including certain imaging studies, also need prior authorization. That means the insurer reviews the request before the service is performed. Our staff submits the clinical information the plan asks for and follows up if a decision takes longer than expected. Approval times depend on the insurer, not on us, and a few requests may be denied or need more documentation. If that happens, we explain your options, which may include an appeal or a different in-network specialist, so your specialist referral in NYC still moves forward.

We accept most major insurance plans, including Medicaid and Medicare. Call your clinic to confirm your plan, and ask your insurer whether a referral is required before you book any specialist appointment on your own.

After Your Specialist Visit

The weeks after a specialist appointment are where care often falls apart: a new medication is started, a test is ordered, and no one connects the pieces. We try to prevent that. When the consultation report arrives, your provider reviews it, updates your chart and flags anything that needs action.

At your follow-up, we go over what the specialist found in plain language, reconcile your medication list and schedule any tests or return visits. If you live with a long-term condition, we fold the specialist's advice into your ongoing plan through chronic disease management, so routine checks can stay close to home. Whether you are seen for a specialist referral in Elmhurst or a referral from Chinatown, the same team keeps track of the whole picture and answers questions between visits. You can also schedule a follow-up appointment online.

Available at Both Locations

Referrals are coordinated at both clinics, Monday to Friday, 9 AM to 6 PM. The Elmhurst, Queens clinic on 75th St has an English and Spanish speaking team, and the Chinatown, Manhattan clinic on Catherine St offers help in English, Chinese and Spanish.
