Information We Collect
When you use this website, City Prime Care collects two kinds of information: details you choose to give us, and limited technical data gathered automatically. When you fill in our contact or appointment request forms, we ask for your name, phone number, email address, the clinic you prefer and a short message. We use these details only to respond to you and arrange care.
Please do not send health information through website forms. Forms are meant for scheduling and general questions, not for symptoms, diagnoses, test results or insurance member numbers. If you need to discuss your health, call your clinic or bring it up at your visit. Information in your medical record is protected under federal law and is covered by our Notice of Privacy Practices, not by this policy.
Automatically collected data includes your browser and device type, a general location based on your IP address, the pages you view and how you arrived at the site. We do not use this data to identify you personally.
How We Use Your Information
We use the information you submit to reply to your message, schedule or confirm an appointment request, and route your inquiry to the right clinic. A member of our front desk team may call, text or email you using the details you provided. If you ask us to stop contacting you by a particular method, we will honor that request.
Technical data helps us understand which pages are useful, find and fix errors, and improve the site for people who browse on phones, older devices or assistive technology.
We do not sell your personal information, and we do not share it with advertisers. We share it only with service providers that help us run the website, such as hosting, email and form tools, under agreements that limit how they may use it, or when the law requires us to disclose it. Your use of this site is also governed by our Terms of Use.
Cookies & Analytics
Our website uses cookies, which are small text files stored by your browser. Some are needed for the site to work properly, such as remembering your language choice. Others support analytics.
We use Google Analytics 4 to measure how visitors use the site, for example which pages are viewed most and whether visitors reach the appointment page. Google Analytics sets its own cookies and processes data under Google's own privacy terms. No health information is sent to analytics: we do not pass form contents, names, phone numbers or email addresses to Google, and we do not use analytics data for advertising.
You can block or delete cookies through your browser settings, and you can install Google's opt-out browser add-on for Google Analytics. Blocking cookies will not stop you from calling a clinic or requesting an appointment, although some site features may work less smoothly.
Your Rights
You can ask us what personal information we hold about you from this website, ask us to correct it if it is wrong, or ask us to delete it. We will respond within a reasonable time and may need to verify your identity before acting on a request. Some information may need to be kept when the law requires it, for example if your message became part of your medical record or a billing file.
New York's SHIELD Act requires businesses that hold private information about New York residents to maintain reasonable safeguards and to notify affected people of certain data breaches. We use administrative, technical and physical safeguards designed to protect the information you send us, including encrypted connections to this website. No website or email system is perfectly secure, which is another reason we ask you not to send health details through online forms.
This website is not directed to children under 13, and we do not knowingly collect personal information from them online.
